How to Set-up an Account
In order to set up your account you must complete and return the following forms to us:
You may mail or fax your paperwork to us. Once we receive all of these forms, you may place your donor sperm order at any time.
Please note, if you do not purchase sperm within a year of your account set-up or your account has been inactive for over a year, we will require a new Authorization for Release form be signed by your doctor.

- LN2 Tank Rental Agreement
