Donor Sperm
How to Place an Order
1. Once you have set up your account (see How to Set Up an Account for details) you may place your donor sperm order. Please call us as 800-473-9601 to place your order.
2. Rank three choices from the donor list according to preference. Use no. 1 for first choice, etc. The ranking of the specimens is advised in the event that a donor is no longer available and an alternate must be selected. We will make every effort to match the specimen according to the ranking you provide us.
3. An advance notice of 24 hours is appreciated for all shipments. Notification must be made to CryoGam before 2:00 p.m. (Mountain Time) the day of shipment. Please note that we only ship orders out Monday through Friday (we do not ship out on Saturday or Sunday), so please plan accordingly. Please see Shipping Information for more information regarding shipping options.
If you would like to pick the specimen up, an appointment must be made prior to coming in. Please call us at 800-473-9601 to arrange a time for the pick-up. Notification the morning of the appointment is appreciated. Emergency weekend and holiday pick-ups are available by appointment only.
4. Payment must be received IN FULL by CryoGam prior to shipping of the specimens or at the time of pick-up. If your physician is ordering on their account, you must arrange payment with them. We accept Visa, Mastercard, Discover, cash, check or money order.
