Long Term Storage
Please contact us at 800-473-9601 to arrange the transfer. Prior to transferring your cryopreserved embryos, oocytes or sperm you will need to complete the following forms:
  •  Storage Agreement
  • Request to Transfer Reproductive Tissue
  • Acknowledgement of Receipt of Privacy Notice
  • Liquid Nitrogen Dry Shipper Rental Form
These forms can be mailed or faxed to you. Please call us at 800-473-9601 or email us at cryogam@lpbroadband.net to request the paperwork necessary to transfer your sperm or embryos to CryoGam Colorado, LLC.

Once we have received payment (see Fees for Storage and Transfer) and all the necessary paperwork, the cryopreserved sperm, oocytes or embryos can be transferred to us in a liquid Nitrogen shipper. If the facility you are currently storing with does not have a liquid Nitrogen shipper we can provide a liquid Nitrogen shipper to facilitate the transfer (additional fees apply).
