Q: What do I need to do when I am ready to use the cryopreserved sperm, oocytes or embryos that I have stored at CryoGam?

A: CryoGam requires you to sign and submit a written request to ship the specimens. We can email, fax or mail this form to you. Please note that this form needs to be notarized, so please plan accordingly. Once we have verified your request we can ship the embryos, sperm or oocytes to the clinic of your choice (packaging, handling, and shipping fees apply).

 

Q: What is the procedure for terminating the storage agreement?

A: A signed, notarized letter of termination must be received from you, or in the event of your death, your legally authorized representative.
